Giving up a dog is a tough decision, and we understand the emotions. Our team is here to support you and help find best solution for you and your furry friend.

Completing this form marks the beginning of surrender process for your dog. By submitting it, you initiate the search for a suitable foster home where your pet can be placed. Please note that the rescue organization must identify an available foster home before the transfer of ownership to the rescue. Your collaboration is essential in facilitating a positive outcome for your dog.

Note: Due to new laws in place in the state of Ohio, all surrenders will be checked with the local dog warden before approval of surrender to ensure there are no documented reports not disclosed on this form - Photo ID of the owner of the dog must be provided to obtain picture and/or documentation for our record at the time of the final transfer paperwork.
